ATS Student-Athlete Portal
ATS Online Portal Instructions for New and Returning Student-Athletes
New Student-Athletes (1st year and Transfers) - click for instructions
· Create new ATS Profile (delval2.atsusers.com Username: new Password: new) and complete all required demographic information
· Upload DelVal Pre-Participation Physical Form Signed by Physician Dated After June 1
· Upload Sickle Cell Lab Results or Newborn Screening Results
· Upload Concussion Vital Signs Baseline Test Confirmation Page
· Upload Copy of Insurance and Dental Cards (front and back)
· If you are prescribed medication for ADHD, upload completed ADHD form found on ATS
· Complete the following required forms in ATS:
o Athletic Consent and Treatment Authorization
o Concussion Agreement
o Medical History
o Sickle Cell Verification Form and Sickle Cell Lab Results
o Student-Athlete Disclosure Form
Returning Student-Athletes - click for instructions
· Log into ATS Profile and update demographic information (delval2.atsusers.com)
· Upload DelVal Pre-Participation Physical Form Signed by Physician Dated After June 1
· Upload Concussion Vital Signs Baseline Test Confirmation Page
· Upload Copy of Insurance and Dental Card (front and back)
· If prescribed medication for ADHD, Upload ADHD Physician Form
· Complete the following required forms in ATS:
